Add Phase 1 (Foundation) implementation plan

Nine TDD tasks: package scaffold, config loading, agentdb bootstrap +
v1 migrations, read-only Firefly client, heartbeat job, scheduler +
healthcheck + entrypoint, verify CLI, containerization, and the
human-in-the-loop production deploy.
